A Daily Fiber Supplement Beat Physiotherapy for Knee Pain, Study Finds

I've spent years paying attention to gut health as its own thing and joint pain as its own separate thing. This trial is a reason to stop doing that. A cheap, everyday fiber supplement just went head to head with a physiotherapy program for knee osteoarthritis, and the more interesting result wasn't who won on pain, it was who people actually kept taking.

The trial: fiber versus physiotherapy for knee pain

Researchers at the University of Nottingham ran a six week randomized trial (the INSPIRE trial) with 117 adults who have knee osteoarthritis. Participants were split into four groups: 20 grams a day of inulin alone, a digital physiotherapy supported exercise program alone, both combined, or a 10 gram a day maltodextrin placebo. Inulin is a prebiotic fiber found naturally in chicory root and Jerusalem artichokes, and it's sold everywhere as a plain, inexpensive fiber supplement, usually marketed for digestion rather than joints.

The real finding wasn't pain relief, it was who stuck around

Both inulin and physiotherapy reduced knee pain by roughly the same amount. But the dropout rate tells a different story: 3.6 percent for the inulin group versus 21 percent for the physiotherapy group, an unusually low number for any home based osteoarthritis intervention. Only the inulin group also showed improved grip strength and lower pain sensitivity, measures that reflect how the nervous system processes pain rather than just local joint inflammation. The inulin group had higher levels of butyrate, a short chain fatty acid gut bacteria produce from fiber, and higher levels of GLP1, a gut hormone tied to pain regulation and muscle health, with higher GLP1 linked to better grip strength.

If you manage knee osteoarthritis and have struggled to stick with an exercise program, this is a legitimate reason to talk to your doctor about adding a plain, cheap fiber supplement rather than assuming physiotherapy is the only path. It's not a replacement for movement or medical care, and it's one study, but the adherence story alone (a supplement people actually keep taking, versus a program people tend to quit) is worth taking seriously on its own.
