It's Muscle Quality, Not Size, That Predicts Heart Attack Risk, Study Finds

The team applied an AI tool to 1,722 coronary CT angiogram scans from the Scottish Computed Tomography of the Heart trial, originally collected between 2010 and 2014. Instead of just reading the arteries, the AI measured skeletal muscle attenuation, essentially how dense or bright the chest and back muscle appeared, which reflects how much fat has infiltrated the tissue. The AI could score this from a single scan in under a minute, versus hours of manual work for a radiologist.

What the study actually found
For every 10 point increase in muscle density, heart attack risk dropped 31 percent and 10 year mortality dropped 39 percent, after adjusting for age, sex, and coronary artery calcium score. Torso and liver fat were not significantly linked to heart attack risk in this analysis. Muscle size, meanwhile, showed no meaningful connection to either outcome on its own. The senior author, Professor Michelle Williams of the University of Edinburgh, and her team published the work in the journal Radiology in 2026.
Honest caveat
This is an observational study, not an intervention trial. It shows an association between existing muscle quality and outcomes, not proof that a training program changes muscle density and then improves those outcomes. The population studied was mostly people in their fifties referred for chest pain evaluation, not a general healthy population, and the researchers themselves say more evidence is needed before this kind of scoring could be relied on clinically. No causal exercise intervention was tested here.
